DEFINITIONS

When CleopatraRX® refers to "Cycle", this means a 4-week period of PearlPAK® dosing that mimics a woman's natural Cycle. It is approximately one month, but it is specifically 28 days. PearlPAK® is shipped in three 28-day Cycles (approximately three months or 84 days). When CleopatraRX® refers to "Subscription”, it shall mean the agreement formed between the user and CleopatraRX®, activated by user’s agreement to the Terms of Service. This subscription is defined by recurring payments according to the structure outlined below in paragraphs 1 & 2, in exchange for PearlPAK® treatment, as deemed medically necessary.

TERMS AND CONDITIONS

1. Payment AuthorizationBy providing your payment information, you are authorizing CleopatraRX® to automatically charge the payment method on file on a recurring basis every 28 days, for a total of three payments per shipment of PearlPAK®. These recurring payments will continue to be charged every 28 days until further notice, or in the event of cancellation, once the payment obligation is met for PearlPAK® that has shipped.

2. Continued Supply:Upon completion of the 12 week supply, a CleopatraRX® medical professional will reach out to confirm your pending refill. If you do not respond to our team's communication efforts within 48 hours, and the prescription is still valid, you will automatically opt in for another 12 week supply of PearlPAK®, and your refill will be shipped.

3. Failed Payment:If your payment method fails, a member of our financial team will contact you via email or text to resolve the matter. After two failed billing attempts, your PearlPAK® subscription and product will be placed on hold until payment is resolved.

4. Cancellation Policy:To cancel your subscription and opt out of future payments, contact support@cleopatrarx.com or call 562-662-7246 before the following scheduled shipment. Any PearlPAK® treatment already shipped will require payment in full. Cancellation will take effect at the end of the current subscription period. Your subscription will automatically renew for another 12 week period, unless you cancel at least two (2) days before the applicable renewal processing date of your subscription. We do not offer refunds for partially used subscription periods, although we may provide refunds on a case-by-case basis in our sole and absolute discretion. We may also offer you the ability to pause your subscription for a specified period of time. If you do not cancel before the end of the pause period, charging to your payment device will resume automatically. We may change our subscription plans and prices from time to time; however, we will provide you advance notice of any price changes or changes to your subscription plans.

5. Refunds:PearlPAK® is a treatment with customized dosing based on each patient's needs. Refunds are not available for products that have already been shipped.

6. Return PolicyWe are unable to accept returned PearlPAK® product. Please discard any unused medication according to current regulations in your state.

7. Shipping Policy The price of shipping is included in the subscription price. We ship via FedEx, UPS, and USPS to the 48 contiguous United States. New subscription products ship approximately 2 weeks after payment is received. The first refill shipment will automatically ship approximately 10 weeks after the first shipment. Subsequent refills will automatically ship thereafter at intervals of approximately 12 weeks. If you need to make an adjustment to this automatic delivery schedule, you must notify CleopatraRX® in writing no less than 10 days prior to the established scheduled shipment date. We reserve the right to increase, decrease, add or eliminate shipping and handling charges from time to time, but we will provide notice of the changes applicable to you before you make your purchase. Any delivery dates or times shown are estimates only and are not guaranteed.

8.Clinical Consultation Fees:The clinical consultation fee is $49. Appointments may be rescheduled or cancelled using the link provided in your confirmation email. Appointments may be rescheduled two times free of charge, but will be charged an additional $49 fee upon the third reschedule. Please note that refunds will not be issued for no-shows or cancellations within 24 hours of your scheduled appointment time.

9. Changes to Terms:CleopatraRX® reserves the right to modify these terms and conditions at any time. Any changes will be communicated to you in advance via email.
